Health is a state of complete physical, mental and social wellbeing. A variety of factors, including environment, economic and social circumstances, as well as a person’s characteristics and behaviours, contribute to an individual’s health and their risk of illness. This questionnaire focuses on health, wellbeing and the way your health affects your life from your perspective. The questionnaire is broken up into specific categories that address the following areas: My Health, My Lifestyle, My Mind, My Meaning, My Work
